Beginning of the Year Survey Questions

I'm trying to piece together a beginning of the year student survey.  What are some questions you ask your students?

  1. Technology at home including computers, internet, game systems, DVD/Blue Ray, smart phones.
    Who taught you the most ever and what did they teach you?
    If you won $1 million, what would you do with it?
    What did your best school teacher ever do that helped you learn the most?
    What are some things that teachers do that are the least helpful in helping you learn?

  2. Student's email address, other activities they're going to be involved in this year, would you describe yourself as a successful Math student & why or why not, what do you hope to get out of this course, do you prefer to work by yourself or with other people, what do you see yourself doing as an adult, in which of the following areas do you do best (classwork, homework, projects, tests)

  3. "The grade I expect to earn in this class is...because..."

    "I learn best when..."

    I second Anna's idea to have them say whether they like working alone or with others best.

    Have you considered having kids complete a learning style inventory? If so, it would be good to have their primary learning style listed on the student survey for quick reference.

  4. I have thought about a quick learning style inventory, but I thought it might overwhelm the students on the first day.
